ADEQ UST Service Provider Program

The ADEQ Underground Storage Tank (UST) Service Provider Certification Program was established in order to ensure that work performed on UST systems is done in accordance with Arizona Revised Statutes1 and Arizona Administrative Code.2 A person must be ADEQ-certified or supervised by an ADEQ-certified person in order to perform certain categories of work on an UST system.

Which tank services require an ADEQ-certified service provider? 

The categories of certification include:

  • Installation and retrofit
  • Cathodic protection testing
  • Interior lining
  • Tightness testing
  • Decommissioning

Note: Collection of environmental samples is not considered as part of ADEQ certification for UST service providers.

How can I become an ADEQ-certified UST Service Provider?

When applying for a category of certification, an individual must submit the following: 

  • A completed ‘UST Service Provider Application’
  • Proof of qualification for the certification category being sought (documentation required varies depending on the category of work, please see each application for more details)
  • Current color portrait of the applicant

UST service providers must also submit a new application form within 30 days following any changes to their application information, including change of employer.3

Amount of Time to Approve/Deny the Application 

ADEQ must approve an application or issue a notice of deficiency within 15 days of receiving it. If the application is incomplete, the applicant has 30 days from receiving the notice to fix the deficiency, or ADEQ will deny the application. 

How do I renew my ADEQ-certified UST Service Provider certification?

Submit a new application form with all the required documentation identified in the application. ADEQ recommends submitting your application at least 30 days prior to expiration of your current certification.

Suspension or Revocation of UST Service Provider certification

ADEQ-certified UST service providers must meet certain performance standards when conducting work. Providers must ensure that UST system modifications, installations, equipment testing, and decommissioning comply with regulations and industry standards and practices.

ADEQ, on reasonable evidence, may suspend or revoke a service provider’s certification if evidence shows they failed to maintain qualifications or exhibited incompetence, negligence, or fraud related to the certified work. 

  • Suspensions last 30 days.
  • Service providers who have been suspended and have another occurrence of not performing tank service in compliance with regulatory requirements may be suspended for 90 days. 
  • If ADEQ revokes a certification, the revocation lasts two years. 

Note: Service providers may appeal these agency actions.

Examples of actions that may lead to enforcement:
  • Performing tank service without a valid certification
  • Failing to conduct work according to applicable standards and practices
  • Falsifying documents to obtain certification
  • Submitting certification statements with false information
  • Failure to follow testing procedures specified by the manufacturer or the identified method.
