This permit is for municipal, domestic, and non-domestic (industrial) discharges of pollutants to surface water that fits the definition of “waters of the U.S.” as described in the Clean Water Act. The permit is valid for five years. A renewal permit can be continued past five years if a renewal application is received 180 days prior to the expiration date of the permit.

Form 1, 2A/2S, 2C, 2D

Form 1 is required for all applicants of individual AZPDES permits, with the exception of treatment works treating domestic sewage (TWTDSs) | View/Download Form >
