UST Service Provider Listing

The ADEQ Underground Storage Tank – Leaking Underground Storage Tank Program established a Tank Service Provider Certification Program in order to ensure that work performed on underground storage tank systems (USTs) is done in accordance with Arizona Revised Statutes (A.R.S.) Title 49, Chapter 5 and the rules promulgated thereunder.  The areas of work affected is defined under Arizona Administrative Code (A.A.C.) R18-12-803 as 'Categories of Certification' and include the areas of installation and retrofit, cathodic protection testing, interior lining, tightness testing and decommissioning.

Pursuant to A.A.C. R18-12-801, beginning from and after Dec. 31, 1996, a person shall not perform UST service on an UST system unless the person is certified or is supervised by a person certified under this Article. The certification requirements of this Article shall not apply to the site assessment or sampling requirements under A.A.C. Title 18, Chapter 12.

In order for individuals to obtain certification in one or more of the aforementioned categories, they must submit documentation as outlined in A.A.C. R18-12-806 | Application Requirements >

Pursuant to A.A.C. R18-12-807(B), a person seeking renewal of certification shall submit an application form and any additional required information in accordance with the provisions of A.A.C. R18-12-806. Pursuant to A.A.C. R18-12-807(C), the UST service provider shall notify the Department of any change to the information reported in the application form on file with the Department by submitting a new application form within 30 days after the change.

Determination that an individual has been certified as a UST service provider in no way constitutes an endorsement of that individual or their associated firm.

The Database Identifies:

  • firms that employ one or more individuals who hold certification
  • information regarding required manufacturer certification for the use of a piece of equipment, product or methodology that is on file with ADEQ for individuals certified in the category of tightness testing
  • information regarding required manufacturer certification for the use of a piece of equipment, product or methodology that is on file with ADEQ for individuals certified in the category of interior lining
