ATTENTION OPERATORS:

The operators listed below have one or more certificates that will be
expiring December 31, 2010.  These operators have until December 31,
2010 to complete training and acquire the thirty (30) PDHs necessary for
renewal.  An operator shall maintain documentation and provide the
documentation to the Department upon request to verify completion of at
least thirty (30) PDHs accumulated during the certification period.  The
certification period is three (3) years unless the operator requests a
shorter certification period in writing.

become inactive on December 31, 2010.  Operators are given ninety (90)
days after the certification period to turn in a renewal form with
thirty (30) PDHs.  An operator shall maintain documentation and provide
the documentation to the Department upon request to verify completion of
at least thirty (30) PDHs accumulated during the certification period.
After the ninety (90) days certificates will become inactive and testing
will be required to reactivate the certificate(s).  If you have already
submitted a renewal form please resubmit the form.  A current ADEQ
renewal form can be found at
http://www.azdeq.gov/environ/water/dw/opcert.html
