The section provides details on how to submit an application for a Certificate of Approval of Sanitary Facilities for Subdivisions (formerly called a Health Certificate). This certificate is required in order to obtain a Public Report from the Arizona Department of Real Estate  . ADEQ Form 222 is the Completeness Review Guide for Engineering Review that provides a quick overview of the requirements for application submittals for Subdivisions, Sewage Collection Systems, and On-site Systems. In addition, an applicant must consult the Delegation Agreement Matrix to determine where the application must be submitted.  As detailed in ADEQ Form 222, the primary components that need to be addressed in any Subdivision application are:
- Final Plat
- Drinking Water Supply
- Wastewater Disposal
- Garbage Service and Disposal
The subdivision rule requires that two copies of the final plat of the subdivision be submitted with the application for approval. This plat must be as recorded, or as to be recorded, in the office of the county or city recorder. ADEQ requires that plats submitted for subdivision approval must at the very least have gone through the tentative plat approval process of the county or city in which the subdivision is located.
A subdivision must contain information regarding the proposed potable water supply to lot owners within the subdivision. This water supply is normally provided by an approved public water system. In that instance, the applicant must obtain an Approval to Construct certificate from ADEQ for the water line extensions to serve the proposed subdivision. In instances outside Active Management Areas where it is not proposed to serve the lots within a subdivision by a public water system, the information required would include, but is not limited to, obtaining a determination of water adequacy from ADWR, and providing information on both the costs to haul water and the costs to drill a well within the subdivision. In that instance, ADEQ should be contacted for further submittal information.
Wastewater disposal in a subdivision must be accomplished either by sewage collection systems or by individual on-site wastewater (septic) treatment facilities. A Construction Authorization is required to be obtained prior to subdivision approval, if wastewater disposal is to be done using sewage collection systems. Wastewater disposal by on-site wastewater (septic) treatment facilities requires a detailed geological report, site investigation, and approvals from the County.
Subdivision applicants must also provide information to ADEQ regarding proposed garbage services and garbage disposal locations.

Where to File Your Application
The Arizona Department of Environmental Quality actively maintains delegation agreements with various counties and cities. This agreement authorizes the local agency to act and process applications on ADEQ's behalf.
