Discharge Monitoring Reports (DMRs)
These reports are required to ensure compliance with Arizona Pollutant Discharge Elimination System (AZPDES).
Self-Reporting Report Forms (SMRFs)
SMRFs are designed for each facility's specific monitoring and reporting requirements as set forth by the facility's Aquifer Protection Permit (APP) or Reuse Permit. SMRFs are mailed to the facility in hard copy.
The SMRF includes the parameters that are listed in the associated permit's monitoring table. The reporting and sampling frequencies and sampling location are listed on each page of the SMRF. For example, if the permit requires monthly sampling with quarterly reporting, there will be three spaces available on the form for Data Entry (i.e., one space for each month). When completing the Self-Monitoring Report Form there are five basic areas that require data. Instructions for completing the SMRF are listed below in steps 1 through 6.
- When completing the SMRF for each of the parameters required by a permit, the block titled "Sample Date" should be completed by entering the date the sample was taken.
- The block titled "Analysis Date" should be completed with the date the samples were analyzed by the Arizona certified lab. These dates must fall within the appropriate quarter to be in compliance with sampling and reporting requirements.
- The block titled "Lab ID" should be completed with the laboratory's identification number, (this number is the lab's Arizona Department of Health Services (ADHS) certification number).
- The block titled "Reported Concentration" should be completed with the reported results of the analysis as reported by a certified lab, if required.
- The block titled "Method" should be completed with the number of the EPA approved analytical method specified in your permit. If your permit allows for ANY EPA approved analytical method recognized by ADHS Laboratory Licensure Rules for the purpose of achieving the detection limit which is equal to or lower than the limit specified in the permit, then the Method number that was used in the analysis should be used to complete the form.
- For any effluent characteristic derived from a calculation using a mathematical average or geometric mean, the date the calculation was made (rather than the actual sample date) should be used to complete the block titled "Sample Date" and that same calculation should be used to complete the block titled "Analysis Date."
If a facility is not producing flow or your monitoring well is dry or not constructed, "NO FLOW", "NO EVENT", "DRY WELL" or "NOT CONSTRUCTED" should be used to complete the SMRF, whenever applicable. If a chemical analysis was reported as not detected (ND), a less than (<) the method detection limit should be used. For example, if the standard method detection limit is 4.4 ug/l and the parameter was not detected, then "< 4.4 ug/l" should be used. When units of measurement in the permit are different than those on the laboratory reports, it is the facility's responsibility to convert to the units that are specified in the permit and as stated on the SMRF. The formula for converting ug/l to mg/l is "0.001 * the reported value in ug/l = mg/l". SMRFs with "ND" as the reported concentration will be interpreted by the Water Quality Compliance Section as invalid.
If a permit contains parameters with the suffix Ethylene, the SMRF will show parameters with the suffix Ethene. Labs are for the most part aware of this.
Furthermore, any area of the SMRF without data will be considered a violation of the monitoring and reporting requirements. To avoid accruing these violations, all areas on the SMRF must be completed.
All SMRFs must be signed by the responsible party. WQCS recommends running a copy before submission.
