The Underground Storage Tanks (UST) Outreach program is a primary point of contact for UST owners and operators as well as citizens with questions and concerns about underground storage tanks. The outreach program produces and distributes a variety of information on UST policies and activities.

File Review (UST and LUST Files)
View public records and files and make a records request by visiting the ADEQ Records Center Monday through Friday, between 8:30 a.m. and 4:30 p.m. You may also make a records request by mail, fax, email or phone.

When making a Records Request, please fill out the form. The form may be submitted by email as an attachment, faxed or mailed.

- (4) The Name of the Company or Group you are representing
- (5) Your mailing Address
- (6) Your Email Address
- (7) Indicate how you would like to Review the Records (Digitally, printed, or in-person)
- (8) Indicate if the Records will be used for a commercial purpose. If for a commercial purpose, indicate the value of the reproduction on the commercial market. (Required)
Help our UST staff locate requested files faster by providing any details you may know concerning the file, for example:
- Current Name
- Address
- Facility ID
- Lust ID

Reminder from the File Room
The Underground Storage Tank Program File Room does not accept reports submitted in a three-ring binder. Please use other binding methods to submit reports, such as GBC Comb or other plastic loop binding, tape or Velo binding.
