Phase I and Phase II Reports
The ADEQ UST program does not require and will not review copies of Phase I or Phase II environmental assessment reports. However, if a Phase I or Phase II investigation discovers a UST release, the owner must report the release to ADEQ within 24 hours. If you wish to obtain a no-further-action determination from ADEQ without the involvement of the Tank Programs Division, please contact the ADEQ Volunteer Remediation Program at (602) 771-4809. Please do not call this number if you are seeking information about a UST or LUST.

ADEQ routinely responds to verbal Phase I and Phase II information requests with information obtained from our database. Although the information may be valid, ADEQ recommends that persons preparing Phase I or Phase II reports review the actual UST and LUST files for factual and detailed information for these reports. The UST/LUST file room can be reached at (602) 771-4344.
